- P2,500 for members in Cagayan de Oro City, El Salvador City and Bukidnon branches
- P1,999 for members in Bohol, and Butuan and Gingoog City branches
- P500 for members with ages 70 years old and above in all branches
Coverage
- P20,000.00 (Aggregate limit per year per insured family)
- Unlimited medical consultation for the principal
- One-time medical consultation for the principal’s spouse
- One-time medical consultation for each of the principal’s children
- Unlimited medical consultation for members with ages 70 years old and above
Qualified Beneficiaries
- Parents of Single Member with ages 65 years old and below
- Spouse of Married Member whose age is 65 years old and below
- Children of Married Member with ages below 21 years old
