MATERNITY CASH BENEFIT

  • A cash assistance given to pregnant members or spouse of the member for medical expenses either through normal delivery or caesarian section with the following criteria:
  • Eligibility:
  • Must be an active full-fledged member for at least 1 year.
  •             In case the member is male, his spouse shall be recipient of the benefit.
  • Member classification of class AA/A or B.
  • In case both the spouse are members of OIC, only one (1) benefit shall be claim.
  • Service Coverage:
  • Every pregnant member will receive a maternity cash assistance and will be given only for the first four deliveries.
  • Exclusion:
  • Miscarriage and abortion are not included in the benefit.  
  • Requirements:
  •   Duly filled-out Maternity Cash Benefit Application Form
  • Photocopy of billing statement/ original official receipt of billing statement for reimbursement or any proof of childbirth such as photocopy of the certified true copy of the birth certificate.
  • Photocopy of member’s valid ID (1 government or 2 secondary ID) with 3 signature specimens.
  • Authorization letter from the member. (if through authorization)
  • Authorized representative’s valid ID. (if through authorization)
