MATERNITY CASH BENEFIT

A cash assistance given to pregnant members or spouse of the member for medical expenses either through normal delivery or caesarian section with the following criteria:
- Eligibility:
- Must be an active full-fledged member for at least 1 year.
- In case the member is male, his spouse shall be recipient of the benefit.
- Member classification of class AA/A or B.
- In case both the spouse are members of OIC, only one (1) benefit shall be claim.
- Service Coverage:
- Every pregnant member will receive a maternity cash assistance and will be given only for the first four deliveries.
- Exclusion:
- Miscarriage and abortion are not included in the benefit.
- Requirements:
- Duly filled-out Maternity Cash Benefit Application Form
- Photocopy of billing statement/ original official receipt of billing statement for reimbursement or any proof of childbirth such as photocopy of the certified true copy of the birth certificate.
- Photocopy of member’s valid ID (1 government or 2 secondary ID) with 3 signature specimens.
- Authorization letter from the member. (if through authorization)
- Authorized representative’s valid ID. (if through authorization)
